What is Acne?

Acne is a very common skin condition, affecting people of all ages, but it’s most common during the teenage years. For many people, it continues into adulthood. In more serious cases, acne can leave behind scars.

Acne usually starts when the skin makes too much oil – sebum. Sebum mixes with dead skin cells and clogs the tiny holes in the skin called pores. When a pore is blocked, a type of bacteria that lives on the skin (called P. acnes bacteria) can grow inside it. This causes the area to become red, swollen, and sometimes painful.

That’s how pimples and breakouts happen.

Types of acne

Acne can appear in different forms, ranging from mild congestion to painful deep lumps under the skin. Understanding the different types of acne is the first step in recognising how best to treat it.

  • Whiteheads: When a pore becomes clogged with oil and dead skin cells, but remains closed at the surface.

  • Blackheads: Similar to whiteheads, but the pore stays open. When the trapped oil and debris are exposed to air, they oxidise and turn dark – just like an apple does when it’s cut and left out.

  • Papules: Small, red bumps on the skin’s surface. These don’t contain visible pus but are inflamed.

  • Pustules: Red, inflamed pimples with a visible white or yellow centre caused by a build-up of oil and infection.

  • Nodules: Large, solid, and often painful lumps that sit deeper within the skin. They don’t contain pus but can be difficult to treat without professional care.

  • Cysts: Deep, pus-filled sacs beneath the skin that are painful and frequently lead to scarring if left untreated.

What causes acne?

Understanding what causes acne helps guide how to prevent and treat it. Acne forms when excess sebum (oil) and dead skin cells clog your pores, creating an ideal environment for bacteria to thrive. Although that combination is at the heart of acne, a number of other factors can contribute to this process:


  • Hormonal changes: Puberty, menstruation, and conditions like Polycystic Ovarian Syndrome (PCOS) can increase oil production.

  • Genetics: Acne often runs in families. If both parents had acne, there’s a strong chance their children will too (up to 75%).

  • Skin cell turnover: Inadequate skin cleansing or a lack of exfoliation can cause dead skin cells to build up and clog pores.

  • Some makeup and skincare items can block pores and trigger breakouts.

  • Stress: Elevated stress levels can worsen acne, as it influences hormone levels and oil production.

  • Diet: Eating junk food doesn’t directly cause acne, but certain components commonly found in these foods may contribute to acne in some individuals. The relationship between diet and acne is complex.

Meet Dr Mark Chernoff

Dr Mark Chernoff earned a Bachelor of Science in Exercise and Health Physiology, followed by a Bachelor of Medicine and Bachelor of Surgery from the University of Melbourne in 2010. He began his postgraduate career at St. Vincent’s Hospital in Melbourne, specialising in general and plastic surgery.

Dr Chernoff serves as a Visiting Medical Officer in Emergency Medicine and works as a GP Surgeon in rural NSW. With extensive experience in skin cancer surgery, medical laser treatments and cosmetic medicine, he also contributes to medical education by supervising registrars for the Australian College of Rural and Remote Medicine and the Royal Australian College of General Practitioners.

Acne treatment varies based on severity and individual skin needs. Treatment options range from over-the-counter products like salicylic acid to prescription medications such as topical retinoids, oral antibiotics, and hormonal therapies. Professional treatments include chemical peels, laser therapies, and microneedling, which can help reduce inflammation, unclog pores, and improve skin texture.

Treatments like salicylic acid, retinoids, antibiotics, or hormonal creams can help, but acne doesn’t disappear overnight. It usually takes 2–3 months to see noticeable improvement and up to 6 months (or longer) for full effects.

Neglecting to wash your face allows oil, dead skin cells, makeup, and pollutants to accumulate, potentially clogging pores and fostering an environment where acne-causing bacteria can thrive.

However, it’s important to note that over-washing or using harsh cleansers can also irritate the skin, leading to increased oil production and potentially worsening acne. Therefore, maintaining a balanced skincare routine with gentle cleansing is essential for managing acne effectively.

And although sugary snacks, refined carbs, or dairy can worsen breakouts for some people, they don’t directly cause acne. Hormones and family history are much stronger factors in most cases.
